Pentair Flow Technologies, LLC located in Ashland, OH: On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created by the rotation of the drag flask. b. Pentair Flow Technologies, LLC located in Ashland, OH: On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created between the pattern lift frame as it passes by the pattern guide wheels. c. Pentair Flow Technologies, LLC located in Ashland, OH: On or about December 4, 2019 the employer did not ensure the Hunter molding machine #2 was properly guarded to protect employees from pinch point hazards created by the pattern and the cope flask as the pattern is raised into position. d. Pentair Flow Technologies, LLC located in Ashland, OH: On or about December 4, 2019 The employer did not ensure employees were protected from crushing injuries created by the Hunter #2 board pusher and the metal conveyor when they manually activated a mechanical limit switch to retrieve more bottom boards.
