Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the controlling employer did not ensure that its subcontractor(s) erected toe boards on all upper level floors and stairwell openings to protect employees from falling objects. The following condition exposed employees to struck-by hazards.
