29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Arbol De Vida located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the employer did not ensure that vertical lifelines used for fall protection were rigged so that employees could not free fall more than 6 feet nor contact any lower level. A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet. Lifelines were not rigged in a way to limit fall distance to 6 feet and or keep the worker from contacting the lower level. The following condition exposed the employee to falls exceeding 70 feet.
