29 CFR 1910.147(c)(4): General - Energy control procedure. 29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a. On or about December 9, 2019, the employer had not developed and documented machine specific procedures for attachments for the double band machine, such as but not limited to the "bunch and cut attachment" (Wohler machine type DSA 92 serial number 2840.) The specific energy control procedure for the Wohler double band machine did not address machine attachments. The employer did not have specific procedural steps for shutting down, isolating, blocking and securing the "bunch and cut" attachment machine during covered activities, such as but not limited to set-up and connecting the equipment to the double band machine, blade changes and unjamming of material from the equipment. On this date, an employee suffered a partial finger amputation while performing servicing activities without lockout/tagout protections.
