29 CFR 1910.147(f)(3): Group lockout or tagout. 29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a. On or about December 30, 2019 and at times prior, employees were performing group lockout and tagout activities on a dust collector such as but not limited to, changing the bags on the dust collectors. This activity was performed without each authorized employee affixing a personal lockout or tagout device to a group lockbox or comparable mechanism. b. On or about November 4, 2019 and at times prior, employees were performing group lockout and tagout activities on Hydraulic Presses such as but not limited to, machine change-overs, tooling repairs and changes, electrical repairs, maintenance repairs, and valve replacements. These activities were performed without each authorized employee affixing a personal lockout or tagout device to a group lockbox or comparable mechanism.
