Site: 118 East 65th Street New York, NY On or about 11/21/2019 a) Employees erected and dismantled a tubular welded frame scaffold without the supervision and direction of a competent person. The scaffold had no safe means of access, not equipped with guardrails, midrails, endrails on any tier, and employees not utilize personal fall arrest.
