29 CFR 1910.303(h)(5)(iii)(B): Entrances to all buildings, rooms, or enclosures containing exposed live parts or exposed conductors operating at over 600 volts, nominal, were not marked with permanent and conspicuous warning signs reading substantially as follows; "DANGER -- HIGH VOLTAGE -- KEEP OUT.": On or about November 3, 2019, on north exterior side of the facility where an employee was exposed to electrical hazards after opening a switchgear enclosure containing 13,800 volt exposed phases that was not marked with warning signs describing the high voltage hazard
