29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Paper Print Department - On or about January 8, 2020, the employer failed to provide adequate training for employees required to perform service and/or maintenance activities on the line 6 press. A machine operator attempted to brush dry ink flakes from the print plate while performing an ink quality check and sustained four crushed fingers and the amputation of four fingertips when his right glove was pulled into the anilox roller. The machine was not de-energized and locked out while the employee was performing this task.
