29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 10/11/2019, during the installation of roofing material at a job site located at 748 Lake Ave NE, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a commercial location (church) were protected from a fall of approximately 9.8 feet by utilizing a fall protection system. The Roof Top Masters LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(11) Duty to have fall protection (Steep Roofs)), which was contained in OSHA inspection number 1366806, citation number 01, item number 02 and was affirmed as a final order on 3/12/2019, with respect to a workplace located at 1133 10th St Ne, Massillon, OH 44646
