29 CFR 1926.1060(a)(1): The employer shall ensure that each employee has been trained by a competent person in the following areas, as applicable: 29 CFR 1926.1060(a)(1)(iii): The proper construction, use, placement, and care in handling of all stairways and ladders; a. On and/or before October 1, 2019, the employer did not ensure that each employee had been trained on the proper use of the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01), specifically on positioning the Tip & Glide wheels to the top of the ladder when used in the straight ladder mode/configuration, thus exposing employees to a fall hazard.
