29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: a. On or about 9/11/2019 throughout the facility employees were exposed to fall hazards of 25- feet while pulling tires from the "E" level where the employer allowed for Turret operators to operate without wearing harnesses. b. On or about 9/11/2019 at location AC 51 B an employee was observed exposed to fall hazards of five-feet, six-inches to the concrete lower level where he disconnected from his retractable lanyard to enable himself to reach tires that were in the adjacent aisle.
