29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. a) At the facility, building 5 rail transfer area: On or about August 31, 2019 energy control procedures did not outline steps for placement, removal and transfer of lockout devices to isolate automatic closing pneumatic gate valves during transfer system cleaning, exposing employees to an amputation hazard.
