29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about 7/23/2019 - 4901 Twin Pine Circle, Hoover, AL, an employee was exposed to struck-by hazards when working in a trench and not wearing head protection. The trench measured approximately 5 to 7 feet deep, 70 feet long, and 3.5 to 5 feet wide with spoil piles up to 5 feet high located on either side and at the edge of the trench.
