29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: The employer is failing to protect employees from caught between, struck-by and crushing hazards associated with the unexpected energization, start up, or release of stored energy when locking out machinery. This was most recently documented at 10330 I St, Omaha, Nebraska on September 5, 2019 when an employee was repairing a John Deere skid steer and the boom arms were not lowered onto the lock pin so residual gravitational energy and stored hydraulic pressure could be relieved and positive lockout could be verified.
