29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Diamond Products, Limited, located at 333 Prospect Street, Elyria, OH 44035: On or about July 15, 2019, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, the Dr. Fritsch Cold Press # 5 (Hydraulic Unit, Type KP 35, Number KP 604) located in the Cold Press Department. Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before conducting activities such as, but not limited to, changing dies. Employees were exposed to caught-in-between hazards that could result in injuries while all hazardous energy sources were not controlled prior to conducting work.
