29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 19, 2019 and times prior thereto, employees were directed to operate, maintain and clear jams in cardboard balers in the cardboard area. The employer did not develop and implement equipment specific procedures for de-energizing the balers to perform servicing such as, but not limited to clearing jams. Employees were exposed to caught-in and amputation hazards.
