29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE): At the facility located at 1301 E. Rd., Fort Worth, Texas: the employer did not ensure a workplace hazard assessment was performed on a roof for proper protective equipment and to ensure employees were protected from falling through skylights by using personal fall protection, protective covers and/or barricades for employees performing maintenance on a HVAC unit. The employees were exposed to unprotected falls approximately 40 feet to the floor below.
