29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished. Location: Worksite On or about August 13, 2019, employees were exposed to hazardous energy in the form of pressurized nitrogen while working on a glycol expansion tank that was not in normal operating condition due to failure of the bladder and was believed, erroneously, to be in a state of minimal or no hazardous pressure.
