29 CFR 1910.178(l)(4)(i): Refresher training including an evaluation of the effectiveness of the training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely: On or about August 7, 2019, the employer failed to ensure employees who operate a propane powered Caterpillar, model number T80C, serial number 55Y00417, were retrained on a three (3) year cycle.
