29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement. 201 Howland Ave, Adams, MA: On August 7, 2019 and at times prior, an employee dismounted a powered industrial truck with the forks raised and loaded with materials. On August 7, 2019, that dismounted operator employee was injured after falling eleven feet while unloading materials from the raised forks.
