29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: On or about February 25, 2019, the employer failed to conduct an audit of their lock out tag out (control of hazardous energy program) at least annually to ensure the program is up to date and effective. The employer has not reviewed their program since the year 2009 and new equipment has been added to the facility, such as but not limited to the Cosen band saw (Saw #01). Employees are exposed to hazards such as but not limited to point of operation and moving parts. -
