29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done: 7605 St. Charles Rock Road, Belnor: Employer is exposing employees to drowning, electrical, crushing, struck by, amputation, and chemical hazards. The employees work at the establishment where hazard assessment of the work place is not conducted. On 07/23/2019 employees were exposed to hazardous conditions such as, but not limited to, working with exposed defective electrical equipment and operating commercial lawn equipment close to a creek, on soggy grounds or inclined environments without having written and certified hazard assessments. An employee who did not have access to a written and certified hazard assessment drowned when he operated commercial lawn equipment close to a creek on an inclined soggy ground.
