29 CFR 1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit-required space: a) Jobsite Located at West Rively Avenue and South Sycamore Avenue, Alden, PA: The employer did not ensure that permit-required confined space conditions were evaluated before employee entry. Employees were exposed to an oxygen deficient atmosphere upon entering a sewer manhole without first testing the conditions inside. Documented to occur on or about 11 JUL 2019.
