29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: On or about July 3, 2019, employees were conducting maintenance on the Siemens Dematic conveyor belt and did not reapply the locks to the make adjustments to the belt, exposing employee to the hazard of being caught in the conveyor belt.
