29 CFR 1917.43(c)(5): Powered industrial trucks were not maintained in safe working order: a) In Shed C, where the Hyster forklift, model number S120FTS, serial number J004V045185, was not maintained in safe working order in that the tires were defective and the fire extinguisher had not been inspected. b) In Shed C, where the Hyster forklift, model number H120FT, serial number S005V03179M, was not maintained in safe working order in that the head lights were broken, the back-up alarm did not work, and the fire extinguisher was missing.
