29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that were related to the safety of the work to be performed was not determined before work on or near the lines or equipment was started. Such characteristics and conditions include, but not limited to 1910.269(a)(4)(i) through 1910.269(a)(4)(viii): On or about June 25, 2019, north side of an electrical pole, employees were running a new electrical line from an ungrounded utility service truck to replace a damaged 14.4KVA electrical line to restore power to local residence.
