29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1): On or about June 25, 2019, in the North Warehouse at aisle 21/22 the Powered Industrial Truck (PIT), YALE Model No. NTA035SB Serial No. B869T01840M, was involved in an incident in which a worker was struck by the PIT and was subsequently hospitalized. The employer did not ensure that the PIT operator was competent to safely operate the PIT.
