29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Line Three Saturated Belt Line; On or about June 11, 2019 and at times prior thereto, where documented procedures were not utilized for the control of potential hazardous energy, employees were exposed to caught-in and burn hazards, in that; the employees were required to open the cover on the asphalt saturation box and reach between rollers to thread paper and or remove broken pieces of paper without the machine rollers being locked out.
