29 CFR 1926.403(b)(2): Listed, labeled, and/or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, and/or certification: a) Ely Rd., Longmeadow, MA: On or about June 4, 2019, an employee was observed using a defiant brand GFCI to power electrical powered hand tools which was plugged into an extension cord, which is not in accordance with the instructions on the electrical device. LaRochelle Construction, Inc was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.403 (b) 2, which was contained in OSHA inspection number1014765, citation number 1, item number 2 and was affirmed as a final order on 2/5/2015, with respect to a workplace located at 50 Greenacre Ave., Longmeadow.
