29 CFR 1926.454(b)(2): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training did not include the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question. Underside of Bridge, State Line Rd and I-90 at Mile Marker 1.8, West Stockbridge, MA On or about June 12, 2019 during three days of dismantling of suspended scaffolds on both sides of the bridge, the foreman and employees involved had not been trained on the correct procedures for disassembling Safway QuikDeck scaffolds. The foreman and employees involved had never been trained on/reviewed the manufacturer's Safety Guidelines or Assembly Guide and were not able to recognize that they were using unsafe procedures and equipment. On June 5, 2019 the second day of the dismantling, two employees were injured when the scaffold shifted while they were dismantling the scaffold using improper procedures and equipment.
