29 CFR 1904.29(b)(2): The employer failed to enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. (a) The employer failed to enter a recordable injury or illness (loss of consciousness and fall from an elevation) on the OSHA 300 Log within seven (7) calendar days. On or about March 25, 2019, an employee was hospitalized after being found on the ground from an apparent loss of consciousness and fall to a lower level. (b) The employer failed to complete an OSHA 301 Incident Report for a recordable injury or illness (loss of consciousness and fall from an elevation) within seven (7) calendar days. On or about March 25, 2019, an employee was hospitalized after being found on the ground from an apparent loss of consciousness and fall to a lower level and the report was not filled out until May 12, 2019. NOTE: Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
