29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On May 14, 2019, the Employer failed to fully document its frequent and regular inspections of the worksite effectively to ensure that employees were not exposed to fall hazards: 1) On top of the tank; 2) On the ground around the 8-foot deep tank pit; 3) On the ground around the 8-foot deep sump pit; 4) on the metal staircase that was being installed and held in place by the crane.
