29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: Naval OLF NEX: On April 23, 2019, employee(s) were repairing a wireless pressurized line leak detector (WPLLD) in a fuel sump. The fuel system was damaged and spraying fuel. An employee entered the fuel sump without being trained on the safe operations for permit-required confined space entry. Subsequently, employees were attempting to stop the fuel leak and unintentionally ignited the fuel vapors. Employees were exposed to fire and/or atmospheric hazards.
