29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Liscio's Italian Bakery, Inc. - Production line area: The employer did not develop and implement a lockout/tagout procedures for machinery in the production line #1 (54" moulded bread line, DPII-26/28/30, 480V), which includes but is not limited to: Chunker, dough lift, sheeter, board feeder, cutting tunnel, dough dividers, proofer tunnel, dough infeed hopper, flour duster, spreading belt, etc. Employees conducting cleaning, unjamming or set up operations did not have specific authorized LO/TO training and/or were not provided with the LO/TO procedures, occurred on or about 04/24/2019.
