29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device. On or about April 15, 2019, in the truss area, the employer's energy control procedures did not ensure that the lock and tag devices put on the Alpine liner saw (Model #276c) indicated the identity of the employee applying the devices; this condition exposed employees to struck-by hazards.
