29 CFR 1910.177(d)(3)(ii): A rim wheel restraining device or barrier was not capable of preventing rim wheel components from being thrown outside or beyond the device or barrier for any rim wheel positioned within or behind the device: a. On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 2 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine. b. On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 1 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine. c. On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the replacement line 2 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine. d. On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 3 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine. e.
