29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about 04/10/2019 in the production area; where employees perform servicing and maintenance such as, but not limited to unjamming machines and changing blades on woodworking equipment such as, but not limited to table saws, planers and joiners without applying a lock to the energy isolating means (usually a disconnect switch).
