29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) On or about April 5, 2019 employees operating powered industrial trucks (forklifts) were not evaluated on their performance at least once every three years when it was found that their training had been completed on August 20, 2015. (b) On or about April 5, 2019 employees operating powered pallet jacks were not evaluated on their performance at least once every three years when it was found that their training had been completed on August 20, 2015.
