29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: a) Facility: On April 4, 2019 and at times prior thereto, the employer had an informal, lock-out system for the de-barker "HOG" electrical energy sources in the motor control center room. However, the employer had not developed, documented or implemented a formal energy control program that would have specific procedures/techniques, a periodic review and formalized employee training. Employees perform scheduled and un-scheduled maintenance on machines including but not limited to the de-barker and chipper. Note) Certification of abatement AND documentation as supporting evidence of corrective action are required.
