29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors: a) On or about April 3-4, 2019- Planer Mill, the employer did not ensure that employees were properly wearing hearing protection devices. An employee operating the Package Maker was exposed to 96.0dB and was observed wearing earplugs that were not fully inserted in the ear canal. b) On or about April 3-4, 2019- Planer Mill, the employer did not ensure that employees were properly wearing hearing protection devices. An employee operating the Package Maker was observed wearing earplugs that were not fully inserted in the ear canal. c) On or about April 3-4, 2019- Planer Mill, the employer did not ensure that employees were properly wearing hearing protection devices. An employee operating the Green Stacker was exposed to an 8 hour time-weighted average of 94.5 dB and was observed wearing earplugs that were not fully inserted in the ear canal. d) On or about April 3-4, 2019- Saw Mill, the employer did not ensure that employees were properly wearing hearing protection devices. An employee operating the Green Sorter was observed wearing earplugs that were not fully inserted in the ear canal.
