29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On or about March 9, 2019 and times prior thereto, the employer had not implemented a permit space entry program for employees performing torque checks and other maintenance on the middle section of the rocket, exposing employees to the hazard of inhalation of hazardous atmosphere.
