29 CFR 1910.28(b)(6)(i): The employer did not ensure each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminate the hazard: a) Amerinox located at 2201 Mount Ephraim Ave, Camden NJ, Building 2, Mirror Line: Employees who walked along the edge of the pit to access the wooden platform where nitric acid and aluminum oxide were added to the tanks, were not protected from falling, occurred on or about 3/21/2019. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
