29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: (a) Department 126, Poletype Division, Regulator Tank Shop, Leak Test and Repair - On or about March 15, 2019, the employer did not provide an employee working at the leak test station a means of fall protection exposing the employee to falls of approximately 12 feet into a water-filled tank.
