29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: The employer is failing to protect employees from the hazards associated with working from an elevated aerial boom lift. This was most recently documented at 4827 Dodge St, Omaha, Nebraska on March 11, 2019 when an employee was operating an aerial boom lift and he had not received training to recognize the hazards associated with this type of activity. The employee received severe injuries after contacting energized electrical wires.
