29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a) Production Department - On or about 3/4/2019 and at times prior thereto, the employer exposed employees to amputation, caught-in and electrical shock hazards in that the employer failed to provide lockout and tagout devices to authorized employees while employees were required to repair machines such as, but not limited to, the Hanwha Corporation HCM-3100- E1-#6 and Hanwha Corporation Box Packing machines that contained electrical (220-volt), mechanical and gravitational energies.
