29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices (RAGAGEP): This violations occurred at the Chemicals, Inc. facility located in Baytown, TX where employees were exposed to fire and explosion hazards from releases of flammable materials when, the employer did not document that it complied with its chosen recognized and generally accepted good engineering practice, including but not limited to, American Society of Mechanical Engineers Boiler Pressure Vessel Code Section VIII (ASME BPVC) and American Petroleum Institute (API) 510 section 7.8.2, when the process safety information did not include manufacturers data report (U-1) for pressure vessels, such as, but not limited to: a. RX-F7 Reactor (P&ID Drawing No. 4 Rev. #3 (Date:04/04/17). b. RX-F15 Reactor (P&ID Drawing No. 2, Rev. #4 (Date:04/24/17) c. TK-739 Methanol Receiver (P&ID Drawing No. 2, Rev. #4 (Date:04/24/17). d.
