29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 02/26/2019, in the Mezzanine Area, employees did not utilize lockout procedures when doing maintenance to the conveyor belts, such as but not limited to, replacing electrical motor, exposing maintenance employees to amputation hazards or fall hazards if belt is energized while employees are standing on the belt. b) On or about 02/26/2019, in the Mezzanine Area, employees did not utilize lockout procedures when unjamming conveyor belts exposing employees to amputation hazards or fall hazards if belt is energized while employees are standing on the belt. c) On or about 02/26/2019, throughout establishment, lockout procedures were not developed for equipment with multiple sources of energy such as, but not limited to, two sources of electrical energy need to be locked out when performing maintenance on the motor on the conveyor belts.
