29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace. On or about and days prior to February 22, 2019: Employees working at the site located at 4026 North Expressway in Hampton, Georgia are exposed to inhalation hazards from chemicals such as but not limited to lead, arsenic, zinc, and propane from burning/cutting forklifts. The employees are required to wear a 3M half face respirator. The employer did not provide a medical evaluation prior to initial use of a respirator.
