29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): A.) U.S. Bronze Foundry and Machine, Inc., 18649 Brake Shoe Road, Meadville, PA 16335, on or about February 18, 2019 and times prior thereto: Employees were lifting a hydraulic counter balance cylinder via a crane and nylon sling from a Bullard 66" Cutmaster mill (machine #1868). The nylon sling failed causing the hydraulic counter balance cylinder to fall, crushing an employee's right hand.
