29 CFR 1926.651(k)(l): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. Inspections were not conducted by the competent person prior to the start of work and as needed throughout the shift: The employer is failing to protect employees from engulfment hazards associated with trenching and excavations. This was most recently documented on or about February 14, 2019 at a jobsite located at or near: Ivyleigh Court, St. Charles, Missouri. The employer did not ensure a competent person inspected a trench prior to the start of work and as needed throughout the shift. The lack of an adequate inspection exposed employees to hazardous conditions while working in a trench ranging in depth from 13.0-14.0 feet, with near vertical walls.
